Our team is standing by to assist you with all of your printing and signage needs. Often times, we will need some additional documentation to better assist you (and you might need some from us)!

In terms of payment methods, PIP Metro Indy accepts all major credit cards, checks, cash and most other ACH/electronic payment methods.

Credit Application

If you’d like to establish credit with us, please fill out the form below and send it back to us however is easiest for you. This allows you and your company to establish payment terms with us, which are typically 30 days from the date of invoice.

Note: All invoices are subject to a 1.5% late fee per invoice, per 30 days past-due.

Credit Card Authorization

Save time on your next order by keeping your credit card securely on file for future orders. We will not run your credit card until the job has been completed and/or delivered, unless otherwise noted on the form.

Blank Tax Exempt Form

If you/your organization is tax exempt, the State of Indiana requires us to have a current year’s signed Form ST-105 on file – and your organization’s relevant tax exemption number – on file to grant you this exemption.

If you/your organization is a non-profit, we are required to have a current year’s NP-1 form on file. For more information on Indiana Department of Revenue rules on non-profit’s sales tax exemptions, please visit this link.

*All tax exempt forms (ST-105 and NP-1 forms) are required to be updated annually.*
